Hydroxide ions are formed when a hydrogen ion is removed from water.
Can a Hydrogen ion be added to an Oxygen atom to from the hydroxide ion?
Or it just doesnt happen?

Hydrogen and oxygen don't occurs as free "atoms", only as molecules, ie H2 or O2.

Also, the H- ion is actually occurs as a H3O+ ion, the hydronium ion. It's hydrogen attached to a water molcule.

I've never heard of it. hydrogen ions and oxygen ions just dont occur in nature.
