How do I apply for a new birth ceritifcate? I assume I need to go to a BDM office, but what with?

I've checked the BDM website and it doesn't help at all.

bdm (births deaths and marriages)... not bdsm O_____O

Why do you need a new one anyway? Are you changing your name or whatnot?

maybe shes lost one

if u change ur name u dont get a new birth certificate...do you?

Goto the BDM office with ID (ie. passport) and they'll reprint one for $25 (probably more since I got mine reprinted 3 years ago).

kayy umm I got mine a few months ago, 100 point ID check...
Medicare, phone bill, bank statement, passport prolly helpful... I think the first 3 is all I took? They also accept payslips..

Also if you need it same day it costs >$50 I'm pretty sure.. or somewhere around $50.

All Birth Certificate applications must be accompanied by at least three forms of identification. This ensures your privacy is maintained and that information is only released to those who are entitled to it.
As you don't have a birth certificate, you need 2 of...

* Centrelink or Department of Veterans Affairs Card
* Security/Crowd Control Licence

* Recent utility account with current residential address
* Bank statement with current residential address
