in the formula....

T^2 / R ^3 = 4pi^ / GM

is R, the ORIBITAL radius? or just the radius of the planet??

Orbital Radius. So centre of mass to centre of mass.

Centre of rotation to centre of mass. In satellites the centre of rotation is normally the centre of the planet so simplifies to what xayma said.
