I'll just give one example to make it simple. For my HSC Ancient exam result, I got 86. For internal, I got 81. My raw Ancient internal mark was 68%, and I came 6/24. That's all the information.

So please tell me hpow the determined both the external as 86 and the internal as 81.

Your external is 86% because that is what the mark you got in the exam was scaled to. Your internal would have been determined as 81% as it must have been the 6th highest mark that a student received in the externals, and as you came 6th in the internals, it became your internal mark.
